Milk of magnesia will neutralize more acid per dose, but it is also a laxative and will cause diarrhea. Tums is calcium based, most people do not get enough calcium anyway.

milk of magnesia. We use this in the hospital but we don't use tums.

milk of magnesia by far is the better of the two.
However I prefer Baking Soda for acid reduction.
