Do you have a custom mix? A lot of places sell their mixes for dietary supplements with your label on them. You will have to find some kind of science lab to do what you want as a small test. This might cost a decent amount but look into chemists and related people in the phone book near you.
Have you tried your local hospital or pharmacy? I dunno if that's the thing you're looking for, but...
It's probably easiest to make them yourself in capsules. You can buy empty capsules and capsule loading devices. Try at Penn Herb (pennherb.com).
