i have been taking loads today and i feel as though i need the loo!.
i still feel that the stool is to hard and large to ease out and it hurts :(

Lactulose is mainly a laxative!.!.!.not a stool softener!.
Laxatives increase the peristalsis (movement) of your bowel, but do not do much to soften hardened stool!. Laxatives can cause some pretty significant cramping!.
You need a stool softener like Docusate Sodium (Colace) according to package directions, and you need to make certain you are drinking plenty of fluids!. You can take this along with the Lactulose!.
As awful as this sounds, you may be at the point where you may need to physically dig out and break up the large mass of stool that has collected in order to help it pass!. you can do this with your finger or with a small spoon!.
I know how uncomfortable this degree of constipation can be!. If you are unable to pass the stool within a day or so, you need to get in to your doctor so that they can help clear you!.
To prevent this from happening again, you need to make certain you have enough fiber, and fluid intake in your diet, if not (and most people do not) I suggest taking a fiber supplement DAILY (like Citrucel or Metamucil)!. Just make sure you take it with a FULL GLASS of water and chase it with another glass of water or you run the risk of further constipation!. Also, drink 1 glass of water for every hour you're awake!.
If constipation becomes a continuing problem for you, please see your doctor to make certain there are no underlying factors causing it!.
