I work on several clients in their teens or younger. Under 18, I do require that you keep your underwear on. Anything else is up to you. I keep all clients professionally draped with a sheet, regardless of what they're wearing.

It is totally up to you - if you want to remain fully clothed, you have every right to do so.
A masseuse would be all right with letting a 16 year old boy wear his underwear for a massage.
