What would be my first step to do?.. Would i need a Note or something from my Primary doctor and then take it to a Local Evaluation Clinic or what?

I think you should ask your doctor. And why would marijuana help you?
Try eating more fruit.
It is cheaper than Marijuana and it won't make you stupid, gain weight, lose your hair, increase breast size, or cause lung and throat problems.
I know, your thinking....It's not as bad as smoking, right?
Just like getting hit by an SUV is not as bad as getting hit by a train.
Go to your doctor and let him/her know that it helps you. The doctor will (if they feel it medically acceptable) give you a prescription and then you can get a card from the state.
