Clarithromycin is converted by the liver to an active metabolite....14-OH clarithromycin, the presence of alcohol interferes with this thus can decrease the effectiveness of the drug. In antibiotics it is CRITICAL that you achieve and maintain a specific blood level of the drug to adequately treat the infection. If you interfere with this you won't get the full benefits of therapy.

Depending on dosage, potentially fatal to mix alcohol with antibiotics.
