Personally I was told that I'd have to stay in the doctors waiting room or treatment room for about 4 hours on the first dose.

There are tons of sites and blogs full of info about it. I did not go through with it, but do a search and a ton of interesting stuff comes up.

You have to have a script. Boxes contain 30 8mg strips.
Cost is outrageously high but worth it if you really want to get off opiates
