Answers:

Nearsightedness which is known as Myopia is a condition in the eye where long distance vision is out of focus due to the lens in the eye and sometimes the shape of the cornea and eyeball focusing objects in front of the retina instead of on the retina.

Farsightedness which is known as Hypermatropia is a condition in the eye where long and short distance vision is out of focus
due to the lens in the eye and sometimes the shape of the cornea and eyeball focusing objects past the retina instead of on the retina.

To correct Myopia a concave shaped lens (thicker at the edge and thinner in the center ) will focus the image on the retina.
To correct Hypermatropia a convex shaped lens(thicker in the center and thinner at the edges) will focus the image on the retina .
