Check out the web site for The Stuttering Foundation at www.stutteringhelp.org as their book "Self Therapy for the Stutterer" help my cousins and uncles who stutterer (stammer is the same thing). They said that it take practice and determination, but the work is well worth it. All of them overcame their stuttering and became fluent speakers except under extreme stress like when my aunt died.
