It may be a result of too much free time, having Anxiety and/or Depression. Also it may be genetics related.

Just turn on the petrol, deploy the choke lever and hit the start button. Allow to idle for a few minutes, switch off choke and check under the petrol tank for surveillance devices and explosives

For me my paranoia started when my worst fears became a reality then in the mists of my grief I began wondering if other things I was afraid of could happen.
I became obsessed with preventing them from happening so I stayed as far away from things that triggered my fears which resulted in full-blown phobias...its a nasty circle.

I didn't hack your account, I swear, I don't know what you're talking about, how could I hack Y!A? It wasn't me, I swear I didn't send those GAP ads after you. The Cold War? What about it? ME? HOW? AHHHHH! I DIDN'T DO IT I SWEAR!

*laughs maniacally*

Experiences, traumas, minsconceptions from childhood, harrassment, assults, and sometimes it's genetics (in which case it is an underlying symptom of mental illness.
I've had severe paranoia, to be honest I still do. It's much better than before thanks to meds. For the longest time I thought I was "normal", even though I thought everyone was watching/analyzing my every move.

It can be symptom of chronic anxiety disorder.
It can be residue left after the anxiety seems to have disappeared. You can believe the one disorder has gone and then one day, you can't walk under a bridge or an overpass, and cranes? Forget about walking past a construction site where there are high cranes. It makes you wonder where it came from. Residue, that's all.

P.S they can't get you if you keep moving.

When you started asking all this questions that were about me... ME ME ME. You can't kill me now. I'll get you first.

Well, paranoia starts when you think they are all watching you, (you know they are watching you, right?)

Then you hear something, something you're not sure you heard, but you just know it was about you and it was bad, (you know they are talking about you, right?)

Then you start to think maybe, just maybe, someone knows about that thing you said (they know), but that isn't enough (it never is).

They start to follow you (did you see them just now?) and they bug your house so they know what you're saying (they can hear you) and the tap your phones (they know what you say and to whom you say it).

Then they take over your computer silently (did you notice your computer is acting funny?) and record everything you type and everywhere you go (they know you go to that site)

And before long everyone is out to get you. (they will, you know.)
