not likely, withdrawal occurs because the body is dependent on the drug. one dose shouldn't cause that if she's been clean for so long.
Yes. It is possible -- you are wise to be so observant, especially if she was using before --now even a little bit could make her sick.
(Has it occurred to you that perhaps she has already been using more than just this one time, too? I have my doubts about that.)

Yes it is possible, if she had been off of it for a long time then did one dose it is more likely than not that she would get nauseas and vomit. Herion users or often known to throw up after a dose even some of the hardcore users.
