I would suggest doing it in a sort of meditative way, maybe before you got to sleep, get rid of all other thoughts and think about yourself leaving, think about it with a good feeling, think about happy surroundings, co mfort, what you might do when you leave, and then also imagine yourself coming back, so you can imagine coming and going freely.

i don't know anything about hypnosis in particular but I know that the mind over matter, imagining thing really helps

Well unless you can find a hypnotist that will do house calls you will need to go out! This could help you to stop the hermit behavior. Who wants to hang with someone that will not leave their home?

There are good books on how to do hypnosis, you could buy one of those to get started.

What worked for me is I made one appointment with a hypnotherapist and during that session he made a CD to take home and use as often as I wanted to. I think that was better than a self-hypnosis book because it targeted exactly what I needed help with.

If you go for an appointment, try and see a hypnotherapist who is registered with the American Psychotherapy and Medical Hypnosis Association.
