How is Narcissistic Personality Disorder generally treated?!

Question: I know there is no cure, but what does counseling / treatment usually entail?

Let's say the individual knows they have it, is not in denial, and is willing to do pretty much anything to try to change their thinking for the better.

That's really hard to treat. Basically you are teaching a person empathy. You teach them to think beyond themselves. Therapy is the best bet and usually entails treatment in a group setting...getting the person to relate to others in a group. But it takes quite a long time to work on those issues. Be prepared to be in therapy long term. Good luck.

NPD is a superior state of being, being "cured" is simply a laughable attempt to get you to admit weaknesses like "empathy" into your psyche to interfere with reason. It is always rational to profit at the expense of others and there is nothing to be gained from contributing to the human race, taking what you can at the expense of others is the sure-fire way to be successful in life and those pitiful enough to let you take from them deserve it for their stupidity. With your superior genetics it is your -duty- to advance yourself at all costs and the rest of the world owes it to you.

No really, I'm playing devils advocate here. If you thought like this, why would you want to be cured? Are you sure this person has NPD? Maybe you should ask a mental health professional to assess them.
