Discharge ( Hospital )?!
Question: Discharge ( Hospital )!?
I am due to be discharged from day hospital for mental illnesses and i was wondering if anyone could help or advice me on what help should i recieve outside in the community and does your keyworker stay with you when you have been dischargedWww@Answer-Health@Com

They must have some sort of plan of continuing the services you need to function in the community prior to discharge!. They must set you up with some sort of counseling to continue observing your progression after discharge not to mention to monitor your medication to insure they are still effective and at the appropriate level in your blood stream!. I am sure the hospital will tell you this before you are discharged!. Make sure you ask them before you are discharged if they don't tell you!.Www@Answer-Health@Com
It would have to depend on the severity of your illness, if you feel you need a key-worker then you should probably have one!.!.!.Www@Answer-Health@Com
