Answers:
Sorry to hear about your emotional problems!. It's nice to hear people like you so much at work!.
This is how I would edit your letter primarily because I think your letter should not divulge too much information about your whereabouts, particularly your Electroconvulsive Therapy treatments:

"To whom it may concern:

Thank you for allowing me to take time off from work!. Recently, I have elected to leave work to attend to some stress-induced personal matters!. It has affected my job performance!. I look forward to working with you again at full attention, hopefully by the second week of July!.

Thanks again!."

Good luck, and I hope your therapy goes well!.Www@Answer-Health@Com

Hmmm, I think you are giving them too much information!. I'd say "This letter is to confirm that I will be taking the following days off for personal leave!. I will be returning from personal leave on July 7th!. Thank you in advance for your understanding!."

Something like that!. Its not your bosses or your companies business that you are having stress and emotional problems!. Mentioning things like that in a work setting can come back to bite you!. I have done this before, thinking people actually cared, and have had such a hard time dealing with people since then, because every single thing that goes wrong after you make a statement like that, they will attribute to an emotional/stress problem, and trust me, you don't want to be seen like that!Www@Answer-Health@Com

Your place of business might require you to have a note from a doctor in order to do this!. For one thing, if you take off with a doctor's recommendation, it is illegal to fire you for taking off!. However, just on your say-so, the fact that you're having emotional problems might make them fish for reasons to fire you!. Not a pleasant fact, but fact indeed!. People with psychiatric illnesses (in the US) are protected from being fired because of their condition by the Americans with Disabilities Act!.Www@Answer-Health@Com

Go with kyukyu99!. Short, sweet, professional!. You don't need to give more info!. It is for under two weeks and can be treated by your boss as similar to vacation time!. If you boss wants a doctor's note, s/he will let you know!. If so, then it's a formal leave of absense and is filed thru your HR department!. Personally, if they make you go that way, I'd consider taking a little more time to make sure it is enough!.Www@Answer-Health@Com

I would ask, instead of confirm!. I would also leave out the "problems" you refer to and say something like you have personal issues you need to address and you are unable to meet your job expections at the moment!.Www@Answer-Health@Com

I agree with Mags628!. However, how can you give a specific date like July 7!? With you having emotional problems you really would not know if you are going to be able to return by then!.Www@Answer-Health@Com

Whoa, you need some editing! First, have you spoken w HR!? There are forms you may be able to fill out, so it won't look like you re just being lazy or dont want to work!. Also, you may want to speak w/yo doc/physician and tell them you are having these emotional issues!. With that, your doc can provide you legitimate note and you, in turn, won't feel so guilty while you are gone, as well as when you come back!. Believe me, tha last thing you need right now is to have your boss on your ***, about missing work!. Not everybody will tell you the PROPER way to handle these things!. I would also advise you speak w/sum1 you honestly trust to re-draft the letter, your personal business is important, don''t let anyone run away wit it!. if you need more, hit me bacc!.Www@Answer-Health@Com
