you would use it to find out info about the client and also to inform others of any findings you have made of the client!.
Health, welfare, communication, finance, likes/dislikes, client contract,
personal information, community access, activity plan!.
6 monthly, but on-going!.

Sections of a support plan will vary depending on the setting and service!.

General thi8ngs they may focus on are: education and training, employment, mental health, physical health, independent living skills, cultural requirements, housing, benefits/income, family and social relationships, self-care, self-harm, alcohol/substance use

Support plans should be reviewed whenever the Support Worker and client feel is appropriate, and depending on the support needs of the client!. If they are low level support then obviously it would be less frequent!.

As for never being allowed to take risk relevant goals and targets should be determined in a consultation between both support worker, client and other agenices where necessary!. The client should be supported towards reaaching these goalsWww@Answer-Health@Com
