It is a tool that enables you to problem solve wether it be a busines propositon or identifing a hazard to persons or places!.
It is a four step plan that works in a cycle and is often looked and and improved upon with regularity
Assess = what is the problem
Plan = How to solve it
Implement = carry out the plan
Evaluate = Check it solved the problem if not start again if so update at periodic intervals
A risk assessment is usually a scored form that the interviewer (who is most normally a professional in thier field) asks preset questions and depending on the answer scores the questions!.
You would use this to minmise potential problems before they arise and effect the solution that would have most benifit to the person!.
The score achieved determines the action that is required
In your example
Caring for some one in their own home
This would all depend on the age, wieght, sex, health, ill health issues, mental health issues, mobility, living environment and avilavility of family or other help, financial staus sometimes has a bearing to play
To illustrate I will give you a scenario
Woman, 74, lives alone, house with stairs, tiolet upstairs no stair lift!. Hard of hearing and wears glasses, just released from hospital after operation to fix broken hip, disorientated to time and place occaisionally at night , suffering long term depression on medication and mild night time sedative, has a history of fluctuating hypotension (low blood pressure), continent!.
Now the hospital nurse before she has been discharched can use one or more risk assessments (scoring tool) to determine whether this lady needs a referral to social services, district nurse input, occupational health services Physio input, Community psychiatric nusre input, or others!.
From the above information the all professionals can determine whether this lady is at risk from any undue harm when she returns home and what assisstance or equipment can be offered!.
Each of the Professionals will have a risk assessment form that will be tailored to thier own area of expertize!. This hopefully will aid in the lady being able to return to her own home and with assisstance lead an independant life as far as she is able

For a simple version, make a table on word (or on a sheet of paper) and put three headings
1 - Risk
2 - Consequence
3 - Reducing the Risk
So, for example, you might cover
1 - Violence
2 - Carer being injured
3 - Ensure that carer doesn't work alone with anyone who has a history of violence or aggression
