If it was a true friend they would be caring and understanding. They might be surprised or judgemental at first but that's becusase they might not understand the disease and it's easier to judge than understand. (:

You would first want to know if they have been diagnosed with Multiple Personality Disorder and are being treated with the right medications and the right therapies that can change her life for the better. If your friend isn't getting the help that you need than you should tell her that she has to get help. It is very difficult to maintain a friendship with a friend who is not or won't be treated.

If your friend is following all of her psychiatrists orders you can be a source of support if you choose to remain friends. It helps to research Multiple Personality Disorder so that you are fully informed and educated in the disorder. You can go to the site nami.org which is the site for the National Alliance on Mental Illness. On their site you can research the Disorder and you can find the phone number for your local chapter. They are there for support, information and they have the best resources for the help that your friend needs in your area. They also have meetings for those with mental disorders and diseases. They have meetings for those who have friends and loved ones with the same. It is all up to you what you want to do.

I would just tell them "How do i know i'm talking to the real you?" lol basically nothing out of the ordinary! They are still your friend no matter if they had all the diseases in the world. They are still the same person. If you suddenly start treating them different and like they're not the same it will only make them feel worse and could lead to very bad depression. She/he simply has an illness. Just have them know you are there for them and try to help them remember to take their medications even if one day they say "I'm fine i dont need them anymore". Usually when they say that, its only because the medicine is working and once they stop the pills they fall back down 10 times harder.
