Answers:

be honest with her. she needs to know so that she can help you

your life is worth something ! your mental health worker needs to have all this information so she can support you i'm sure you are not the first person who wasn't totally truthful just explaine when you next see her that you felt it was a very personal question and didnt feel ready to answer it then but you have tried to kill yourself before i'm sure she will be ok with this and wont even mention about you not saying anything last time [- it is important you mention it though so she knows how best to help you - good luck and hope i helped

trust me its not worth it... if your willing to just take your life, its because its not worth anything right? make it worth something embarrass your self have fun do drugs were all going to die wait for your time until then try to have fun
