Question: What is wrong with her? Is she Depressed?
She came home and cried every other day whenever she looked in the mirror because all she saw was 'ugly' She started caring less and less about her appearance, she doesn't care if her nails are painted or not. She likes wearing sweatpants and hoodies because she says, "I'm already ugly anyway, why dress up when people will still see me as ugly? " Sometimes i get worried because she says she wants to die because she doesn't think she has a purpose to live in this world & that she would much rather be happy dead. The only thing she has kept maintained his her grades which are A's and B's
She doesn't tell her parents what's wrong with her because it worries them when she's sad...
She keeps everything inside and she gets angry because she's holding alot of grudges on people...

Sounds like she is, she should tell her parents and her trusted friends. It sounds like she is in high school, I would like to inform her that it seems like all of that crap is so important now and it is a big memorable part of your life but after a year or 2 it WILL seem completely insignificant, and the people that act like jerks in high school end up making up be lowest levels of society in the real world, while clever people like her will have happy, fulfilled lives. Tell her to keep on fighting, to try not care what people think or say of her (I realize just how hard this can be), this is her life only her opinion matters.
PS. If this girl looks anything like your profile pic she is far from ugly.

That girl sound like my friend. I know this isn't about me but, I know what you're dealing with. See I have this friend who is "thicky thick" medium , Good Looking. Isn't fat at all. She's beautiful " No homo". She have great curve's. And some times, At school kids pick on her and call her fat and she says she is ugly almost every minute. Even when were not suppose to be talking and she ask the teacher that too straight up. And then, If they answer No . Which is true she isn't ugly. She runs out of the classroom and every one looks at me like what's wrong with her? Then. She don't tell her parents about what's wrong with her. She used to cut her self in the bathroom. Allot in the 8th grade. Then she cut her self on the neck. And, She makes up excuses for them. Then, She is like REALLY REALLY REALLY Depressed. She says she wants to kill her self now and she hate her life. She's not concided and she want's to be a singer one day. I mean. Yeah. So, But you're friend. She might be depressed. I Think you need to like talk to her one day. And see wjhat's wrong and don't tell anyone. But if it get's to risky. You need to tell. You know what I'am saying?
