I know exactly how you feel as I've too struggled with this.
My friends and family members use to give me advice like, “just get it done.” But that never seemed to work as a long-term solution.
I first had to identify why was this happening: was it because I'm over committed or over scheduled? Just lazy? Or simply not interested?
What I learned was that in order to free myself from procrastination, I must always identify the tasks, identify the reasons I avoid those tasks, and then remind myself that it only makes matters worse to put things off. Managing my time more effectively also made a BIG difference.

You get off the internet and just do it. You remind yourself that if you waste time and don't do it you'll fail the project with an incompletion. Just force yourself to do it because you have to do it sometime and it's better now then never. ^-^
Stop procrastinating and start on the project.
Just stay focused. Tell yourself over and over again that its important and you need to do it. Get it done and then reward yourself.

The way i handle it is i keep on telling myself- ''I live NOW''. So..i do what ever i have for me to do now.
Its kindda hard , i mean nothing really practical way to force you to do things on time unless you tell yr subconscious mind so.
Maybe u can try start get a nice comfortable chair , sit properly and put yr hands on yr chest and say yr name followed by ''I will do ...Now''..
Or u can stare into yr own eyes and shout the same phrase.
Start slowly...u wonnt change in a day.
Also get into a group of people who do things on time. U know...who always have something to do in their life...like always busy with something. Therefore it will remind u that u have things to do, and u will do it not in last minutes.
Or maybe....just be spontaneous. Like dont think whether u wanna do it when (usually u will say i'll do this later'' into just do it . No need to think. Take yr work/project and do it now.
Ok..I dont know..this may sound funny to you. But It works just perfectly fine for me.
