How do I fix this sleeping problem?!


Question: How do I fix this sleeping problem?
This problem that I have happens before I sleep and RIGHT when I wake up. Before I sleep I would have these thoughts that get me excited (whether it be sexual or fantasy like) to the point where I can get nervous and my heart beats real fast. These thoughts disrupt the peacefulness I need in order to sleep so every night I would get in bed at 9 and sleep at like 11:30. This also happens right when I wake up. As soon as my eyes open, I would automatically think those thoughts and I would not be able to go back to sleep. It really sucks on the weekends where more sleeping would do me good. How can I clear my mind and I mean CLEAR my mind so that i can concentrate on sleeping and sleeping only? Do I have to change my whole mind set? I lose focus easily. It's not because of a.d.d. but something I made a habit of because I was bored when i was little. Share your tips thanks.
