if you had a death of a loved one and you started to have random memories that were triggered at anytime would that be an example of a relapse?

That wouldn't be a relapse.
usually 'relapse' refers to returning to a certain behavior that is maladaptive--such as alcohol relapse, or drug relapse.
Some people stop taking their psych meds because they are feeling better, and think they don't need the meds anymore. that would cause a relapse in depression, Bipolar, etc.

a relapse is more for something bad that you fall back into. For example someone my relapse back inot drugs or alcohol. They also my relapse back into a bad state- such as depression. I don't think a memory trigger is considered a relapse... I think you stated it correctly it's a trigger of a memory...
its like when you have an addiction to drugs, then you go into rehab and then your sober. A relapse is when you go back onto drugs after sobering up
