Answers:
You should be able to make an appointment yourself unless its an issue with insurance or payment. Other options might be through your school. At my college they offer counseling services for anxiety and depression through our health department.
Often there's community programs. Planned Parenthood sometimes can help put you in touch with support groups. If worse comes to worse seek out some of the support groups online until your able to get help in person.
I had terrible anxiety when i was your age and no insurance. It was really rough. It does get better though. Just hang in there and be persistent in getting the help you need.
Writing down everything it helps a lot.
