Answers:

I can understand how you are feeling. I suffered from the same problem for years. It really interfered with my life.
I went to a psychotherapist and it didn't help me. Here's what I did that worked:
I found a friend who was sympathetic and supportive. We drove to the bridge I feared the most......(see pic )http://www.texcote.com/pages/engineers.h...
I took a deep breath and started driving. I was afraid I was going to pass out. My leg was shaking so that I could hardly keep my foot on the pedal. My friend just kept encouraging me and telling me we were safe. I drove back and forth across the bridge four times. Each time, I became less anxious. The cycle of fear was broken. Now I can drive across a bridge without a hint of nervousness.
If this worked for me, it may work for you. I hope so.
