You do not have to divulge any medical information that you do not want to.
Is your ADHD being treated? If it is call your doctor and let him/her know that it is impacting in a negative way in your job and you need additional help with this.
If your ADHD is not being medically treated, then see your doctor and Begin treatment and again, be sure to tell them that is it negatively impacting your work which then in turn also creates anxiety for you.
So before you tell your boss, ask yourself:
Do I want to tell my boss I have ADHD?
Do I need a doctor to help me manage my ADHD?
Do I need to tell my doctor that I still have ADHD problems?
And any other question that you can think of.
Then act appropriately on your answers.
