Should turning them into the doctor be the first option? That sounds extreme for the first step. Like being Big Brother, being self-righteous and judgmental, playing God. Perhaps let him or her know that you realize they have a problem, are addicted. Gently point out the ways the addiction is hurting them, you, and others. Lots of times people don't realize the harm they're doing until someone tells them. Especially if they think noone knows. Offer them support, compassion. Your friend could be desperate to get off the pills, just is afraid to confide in someone, and doesn't know how to go about it. Offer to take them to a 12-Step meeting. I've seen love do wonders, and scorn really hurt the user. Remember, they are sick in some way. And until the underlying root of that sickness is treated, they will return to that drug, any drug, to kill their pain. Resentment to you being a tattle-tale can drive them into using street drugs, especially if the doctor abruptly cuts them off. And you will lose all effectiveness at helping them. Later, if you've had the talk with them, and you've offered your help, and they are just getting in deeper, more extreme measures can be taken.
