They're not showing a 'man who used to be a woman'; they're showing a man who was born transsexual, and has had/is having that congenital condition treated.

no, they construct a penis from vaginal skin and nerves. let's face it she wont need her vagina any more. they use that existing tissue to fabricate a penis. it doesnt become erect, though they can do an implant that works with a pump, naturally it doesnt ejaculate either, though they can sometimes achieve orgasm because they use the clitoral nerves so they have some sexual functiion in many cases.
