Well....stress and fatigue are two major factors. Also if the guy ejaculated even once earlier that day, may cause a delay the second, third....etc time around. If he is on medication other than a male enhancement drug (i.e anti-depressants and sleeping pills have been known to inhibit arousal). Other than that, your guess is as good as mine. :)
Purposely wanting to take his time, to get a stronger orgasm. The longer you wait, the stronger the orgasm is.
Also, maybe he is so used to masterbating, that he can't get the same 'up and down' speed from normal sex.
