If it occurs during birth, it could be caused by a lack of hormones. In this case a hernia would be present (and a minor operation would be present shortly after birth).

If your testicles just hang low, and will not retract during different temperations, you may be suffering from a nerve injury (a injury to the L1 nerve).

You may be lacking the cremasteric reflex.
