It can be both. Some men have a large scrotum with a thick penis resting upon it. Others have a small flaccid penis (and small is normal, it is okay for a man to be totally soft) with the small penis resting on the big pillow. Most men's scrotums are average size or smaller when relaxed. (Thank God we can relax it) . Some men just have an average pillow and average penis. Some have a small relaxed penis with a small relaxed pillow.
It could also be the material of the pants and the air pocket inside.
Really it depends on where the penis is situated. To the left? To the right? Or does it go over or under? Some men perfer to situate their penis up so that its not resting upon the scrotum. Type of underwear is a factor too.
It can mean either or both. Most guys I've seen have either one or the other that's big, but some have had both. My ex had a rather thick and long penis, but his balls were tiny. Guess it all just depends.

umm..i have a big ball sac..thts why my penis looks pretty big..but its average size and when i play sports..when it shrinks..because of my big balls..they would just appear on the trouser..and thats only because i have big balls..to answer ur questions..its because guys have big balls when they appear outside of there pants...hope i helped
