You don't have a disease, you're perfectly normal. Right now, you're starting our puberty, right? Well, your body is still getting used to getting erections, so things might be a little different now than how they will be in a few years. You have a lot of blood rushing to your penis when you get an erection, therefore your erection is strong enough to be "bent" upwards. There's nothing wrong with you, as you get a little older, it will slowly get a little lower. Since you're only 12 and probably aren't thinking about having sex, there's no reason to worry. You're going to slowly see your penis looking less and less bent, until it gets to where it should be. There's no way of getting it perfectly straight, although most guys don't have a straight penis. Some curve up, some point up, some curve to the right, etc. Everyone's is a little different.
To answer your size question...5 inches is very good for someone who's 12. I remember when I was 12, mine was like 3.5 inches long when it was hard and I was happy, lol. Your penis should stop growing around age 21, and since you're just starting puberty, there's growth left for you to do there. You can't rush puberty, you can only survive it.

Good luck, hope I helped! Feel free to message me anytime if you have anymore questions at all. =]
