What do you mean "sound painful"?? It is painful.....right? Usually this expressions are invented by neighbourhood gangs. The guys from the gang try to seem like cool and insensitive. That's why they are using a lot of violence in them expressions.

As a complete outsider, it definitely looks like a violent act, especially the way some guys do it. It's done to ejaculate and it doesn't need sweet-talking to get it in the mood. Violent names that refer to male masturbation seems extremely fitting.
Concerning female masturbation, there are names like "beating the beaver" and "tickling the taco". Those don't seem as violent but remember, according to some people, women never do things like that.
