Question: Even after he showers, I tell him to scrub it real good with the loofah and he insists he does. I can smell it when he takes off his underwear. It smells rotten. I call it rotten semen. Could he have an infection or is just due to improper washing? I could NOT even consider going down on him with it like this.

if he showers daily and after each and every activity (like running or sport or the like) then he should not stink. though if he has foreskin and hasn't learned to retract to clean under it that may be the source. So, check him out and if the foreskin is there watch hiim shower to see if he retracts to clean. If not tell him to. If he cannot retract, then he needs to learn to stretch the foreskin so it will retraxt.
If he is circumcised or does retract to clean and heshowers daily and more, but his penis area still smells then he undoubtedly has some kind of bacterial growth that is going to require a treatment to fix. It may be as simple as a yeast infection (so don;t start thinking STD because that is probably unlikely)
