i had a simialiar problem like his, at night i was awaken to excruciating pains in my testicals. It went on for about a year before i went to the doctors. It turns out it was a serious problem i had a vein or something twisted and it was cutting blood off to my testicals
He might have a hernia, my brother got one when he was 2 just from trying to take a poop.
This isn't normal. If his testicles hurt badly enough to tell you about, and it hasn't developed into the flu, you should have him checked by a physician.
