I am a massage therapist, you don't have to, it's whatever you are comfortable.
Less is best, but when it comes to underwear, you can have it on and it wont make a difference.

No. You take off everything but your underwear, then lie on your stomach. They have a sheet they drape over your lower half.

If there are no laws or policies prohibiting completely undressing, you should undress to your comfort level. For some people this means leaving underwear on and for others it means taking everything off.
