actually, you can have surgery to make it bigger!. there are a couple of ways it's done:
One is to cut the ligament that holds the penis to the body, and allows more of it to come out of the body!. The problem with this surgery is that now there's nothing there to hold things in place, so the penis can very easily slip up inside the body!. In order to 'train' it to stay out, you have to wear a weight attached to your penis for about a year or so!.
Another way is to inject collagen or other material into the penis!. The problem with this is that while you'll look bigger flaccid, it doesn't do much for the erect state!. There's only so much spongy material inside to fill with blood, and so much skin on the shaft of the penis itself, so you're kinda limited as to what it can do and how much you can 'grow'!. yer you can get an enlargment tho not on the nhs,i think you were asking the question,why can you only get the chop on the nhs but not enlargment!.
the answer would be,if its smaller than normal ,the nhs doesnt think its a real problem,but if its too large,you and your partner would both suffer!.

There are both -- but operations to make a penis bigger rarely add more than a quarter inch -- and sometimes fail -- thus actually shrinking the penis they are trying to grow!.
