I'm pretty sure it doesn't affect fertility!. Obviously the veins get so enlarged because extra blood flow is going to the penis, and it's suddenly being stopped, so more blood will flow through at a greater rate!. depends on how he does it there are devices sold that are for that purpose but they advise that the ring is not too tight and that it is not worn for extended periods of time!.
