Question: Pls Help: Circumcision OR Preputioplasty?
I am a 21 year old Hindu male. I have complete phimosis and frenulum brevae since birth. I have now tried stretching for months with steroids but to no avail. So my question is should I get circumcision done or just have a preputioplasty. Aesthetics is also a factor. I have seen images of dorsal slits and rather than having such jagged, hanging weird looking foreskin I would prefer it to go away altogether. So, I will consider prepu.... only if it results in completely normal looking uncircumcised foreskin. Also, since my glans has always been completely covered it is extremely sensitive. Even a slight bit of touching the head hurts like hell. Won't circ decrease the excessive sensitivity and make it normal ? Circumcision is very uncommon among Hindus so I should better take this decision very carefully. Pls answer if you have anything useful to say. I don't want only anti-circ rants.

P.S: I had attacks of paraphimosis (OUCH..) twice in the last 12 months so something needs to be done now.

Keep with the stretching.
Circumcision is one of the worst ways to self-mutilate yourself.

Your penis head (glans) will toughen and become less sensitive and sex will be less satisfying for the rest of your life.

You can stretch it, you just need to try harder, I know it's difficult in your case but trust me it will be worth it.



Even given the severity of your issues with your foreskin, you would very much regret getting rid of it. It's the most sensitive part of the penis and having it removed will be very uncomfortable. A dorsal slit would be the best solution.



It seems to me that you researched this predicament of yours already and the final decision is up to you. From what I read, I would suggest preputioplasty. It is a lot less invasive.



Since you are suffering from both of the developmental abnormalities of phimosis and frenulum breve you will be best off having a complete circumcision.

In theory you could have a preputioplasty and a frenulectomy but there would be little to gain and the frenulum would heal very slowly since it would be kept in the moist cavity of your foreskin and deprived of direct contact with fresh air which is needed to aid healing.

A preputioplasty starts with a dorsal slit, but unlike the so called dorsal slit circumcisions, which leave a hanging tassle of skin, in a preputioplasty the slit is quite short and the edges of it are sewn back together transversely, thus widening the opening. You will end up with a horizontal scar across the foreskin near its tip. This can be harder to explain away than the circular line around the shaft that a regular circumcision produces.

Although circumcision is not praticed as a religious ritual by Hindus, it is far from unknown for Hindu men to be circumcised. Indeed, with the current knowledge of its prophylactic effects against HIV/AIDS, circumcision is being chosen by a growing number of intelligent males of all religions.

Circumcision totally cures/prevents phimosis, paraphimosis, frenulum breve and balanoposthitis. Indeed it is the only way to ensure that a paraphimosis will not occur again once one has suffered once from this condition.



You know, it is so silly to think that you have to do things to hurt yourself or to be like somebody else. Just because someone says you have to retract your foreskin why do you think you have to do it? The reasoning is not correct that if a foreskin isn't retractable it can cause problems with cleaning and sex, and if it doesn't retract it's not functioning properly.

There actually are millions of men in the world at any one time, myself included, who do not retract their foreskin. I have a number of current patients in a similar condition and have, in the past, treated men in Europe, Asia and Australia with the condition of tight foreskin. Not one that I have personally met and not one that I have ever read about has a problem with cleaning. Nor do they have problems with masturbation or sex. And I have to join with them in saying that it is ridiculous to suggest that our penises are not functioning properly.

Your penis and your foreskin are a unique team. They work together almost always in perfect harmony. Some guys have a foreskin that retracts, so they retract it. Some guys have a foreskin that does not retract, so they leave it alone. Some guys have had their foreskin removed and no longer have a choice. If you are watching videos, then the guys are probably porn stars and they do so much drugs and other stuff that their sexuality is no longer normal in any case. I would not worry about them.

If your foreskin does not easily retract, do not try to force it to. It does not have to. You do not have to pull the foreskin back to put a condom on, to masturbate, or to have sex. It is an absolute nonsense to think so. Only people who really do not know will tell you this is necessary.

Some people label a tight foreskin as phimosis. Generally people with a tight foreskin do not have phimosis, they just have a tight foreskin. There are millions of guys in the world with this condition which is quite normal. The only abnormal thing is when you try to stretch a foreskin that does not need to be stretched or try to use medications that rarely have any beneficial result.

The unfortunate thing is that retraction has been taught, mainly in USA, because personal hygiene was not taught. Washing a penis with the foreskin covered (not retracted) is very easily achieved and very efficient. Any boy or man can be shown how to do it if they are having a problem. The same goes for masturbation or sex. Many will argue that it is much more effective and it certainly feels great. I cant compare it to an open foreskin, because I have never done it that way.

Just leave your foreskin as it was intended to be. Anything you do to yourself that hurts is normally not a good thing to do, so don't do it. That particularly includes the terrible mutilation of circumcision which will leave you damaged for the rest of your life. If you’ve never pulled back your foreskin, then just don’t be conned into thinking that you have to. Some people do because they have a loose enough foreskin to do it, and some people don't because they have a tight foreskin. It is a simple as that. There is no right and wrong about it. There is nothing less normal either way. It is just how things are. Some people force themselves to retract by stretching and using creams and all sorts of stuff. It is ridiculous and has the distinct possibility of causing life time damage. Don’t ever do it!

What you need to know about circumcision is that is it a terrible mutilation of the human body. No civilised person could ever allow this to happen to themselves or to someone they claim to love. Circumcision is the removal of the (male) foreskin. Uncut or uncircumcised refer to the natural state of the penis where the whole of the sexual organs are intact. About 90% of the males on Earth are not circumcised, but it is still practiced as a routine in Israel and in the USA.

It is as good to be circumcised as it is to cut off your eyelid so your eye no longer has the protection and lubrication it was designed to have. A male was born with a foreskin for a reason, and it was not so it could be cut off! Circumcised penises are no cleaner than a natural one - any one who says so obviously has no idea what they are talking about.

Circumcision of the penis does not prevent cancers, HIV, spread of AIDS or any of the myriad of other things that seem to be attributed to it. Circumcision of the male is just as barbaric and cruel as circumcision of the female. Most people would not even consider inflicting such damage on a girl, yet some still consider it an acceptable practice to perform on a boy.

If you have retracted enough to have had the foreskin trapped behind the coronal ridge, you do not have phimosis of any kind. You have a naturally tight foreskin that just needs a bit more stretching.

In short, I will say the foreskin contains more than 20,000 touch and pressure receptors that represent 75% of the sexually sensitive tissue on the penis. How could letting some hack in a lab coat slice this off make things better? It can't and it won't
If your doctor has not prescribed Betamethasone steroid cream to help loosen the tight ring of muscle (Frenar Band) that closes down the skin opening, then ask for it. The last thing you need is to have someone take a knife to your penis.
I have attached a few links below that will give you information about how lucky you are to have a foreskin. Circumcision, by the very nature of it, is a highly destructive procedure that unnecessarily removes healthy, vital, living tissue for no good reason. If you have to have surgery at all,...go for the preputoplasty and retain the foreskin.
I have been restoring my foreskin for four years now so I can speak from both sides of the table on this one. The foreskin I restore will be a poor second choice to the one that was hacked off my protesting body all those years ago, but it will be far better than living out my life without a foreskin.

That suxx. I defiantly recommend the Preputioplasty over a circumcision. A Preputioplasty will give you a normal looking and functional foreskin

sometimes it is considered normal for a 21 year old to still have these growing issues, you might grow out of it with out any intervention.

I recommend that you do what ever you can to keep that tissue as its the most sensitive.

Have you made any progress with he stretching? you may want to consider trying a different technique. There are also stretching devices such sometimes with weights you can wear all day.

As for your frenulum brevae. If stretching does not work, you can always get it severed. This way you get to keep that sensitive tissue but you need not worry about it breaking as it would not be connected any more.

When males are born their foreskin is ATTACHED to the head of their penis. That's not phimosis, it's normal. So you haven't had it since birth, just since your teen years.

If you are doing the stretches properly, and often enough with the steroid cream you should see slight results at the very least. You really want to do the stretches 10 minutes at a time and 3 times a day. It's best to do it with an erection, pull the skin back as far as you can without pain and hold it there for the full 10 minutes.
Also I don't understand how you can know you have a frenulum breve if you can't retract your skin even part way. Usually it's going to be one or the other, not both.

The reason why touching the head hurts really bad is because #1 it's been covered for 21 years, and #2 it hasen't been cleaned in 21 years. So those two combined are going to make the head VERY irritated and sensitive. The way circumcision reduces sensitivity is the head is left dried out and exsposed. It will be very painful for a month or so while the head is rubbing against clothing all day. Then sensitivity in the head will deminish, however what sucks about this is even the positive sensitivity is lost over years and years of the head being exposed. You don't just lose hyper sensitivity you lose sexual sensitivity and that adds up over a long period of time.

It's up to you what you do, it's your body and you know what's best for it over anyone else. Seems like you have tried a lot and you really just want this over with. Get a really good physical exam from your doctor. I find it really hard to believe you have both conditions only because it's so rare. If you just have a frenulum breve then just have the frenulum cut (frenuloplasty) and then your problems will be fixed. Really the only time a preputioplasty works is when you have a phimotic ring and only the tip of the foreskin is tight. Not when the whole thing is tight. So if your whole foreskin is tight that's not an option since it's not going to fix anything.

I advise most guys not to get circumcised, in your case it sounds like the best thing to do.
Talk to your Dr. and ask about a loose circumcision, some doctors take off a lot of skin and it makes a guys penis uncomfortable during an erection.
I was circumcised about 30 years ago, the doctor did a loose cut, some skin still covers my glans,
It will take a while for you to get used to having your glans exposed, eventually it will become less sensitive, I hope this helps, good luck.



Inform yourself very thoroughly before deciding. Remember that the rugeon has a good reaon to propose circumcision, namely his fee.
But if the operation goes wrong, which it does sometimes, they cut everything off and turn the victim into a transsexual “woman”, as has happened in several cases, most famously with David Reimer.
Circumcision will make the penis extra sensitive for a few months because the glans is exposed. But afterwards the feeling in the penis is greatly reduced: some two thirds of the nerve-endings in the penis are removed in the operation.
Then scar tissue forms, making the penis less sensitive. The penis rubs against your leg and pants, so loses even more sensitivity. Callussing sets in.
At a later stage keratinization takes place and it becomes very difficult to reach orgasm or even keep an erection without medication.
