for the female anatomy, you just add/subtract flesh.
for the male anatomy, there are several factors that play. You cannot just add flesh or fat (it might end up in lumps or it might disfigure.) Then it must also work during erection. The penis engorges and turns flaccid. It is difficult to substitute for something like that. Surgery sometimes leads to bad results, and it is irreparable for a guy by then. (Bad results even happen with females right?)
for females, some of them have that nice look but bad feel. It will still be okay for them. for guys, it must look nice and it still must WORK! We use it for urination, procreation and recreation. d:
Add: guys are mostly visual, so the looks are priority. The improvement to the female is on the whole visual -- an attractor to most guys.
girls are sensual (the sense of feel), so making it look big will not attract most girls. Also because of that guys with good looks sometimes are not necessarily the top ones. The best advise for guys is not to capitalize on the look/size of it but to capitalize on how they use it -- that's when the girls can feel it.
