Answers:

The foreskin should go far enough back to expose the entire glans (penis head). The foreskin should be attached to the glans on the underside via the frenulum and that's normal (link 1). In some guys the frenulum is a bit short and makes it hard to pull back the foreskin, but by gently retracting the foreskin as far as it'll go, you can eventually stretch out the frenulum. Sometimes there are adhesions attaching the rest of the foreskin to the glans, but moving the foreskin around and proper hygiene will help loosen those adhesions until they detach.

But what it seems like you might have is a condition called phimosis, where the foreskin opening is too tight/narrow for you to be able to pull back your foreskin. Fortunately this is pretty easy to take care of in most cases, it just takes some time (see links 2, 3, 4, 5).

In any case, you'll probably encounter people who'll tell you to get circumcised but it's almost certainly not necessary, so ignore them unless everything you've tried has failed (and that's pretty unlikely).
