If it doesn't slide back, they'll rip it off from the head, after all, they're going to cut it off. That's what they do to babies (although circumcision is now a lot less common), since on newborns (and young children) the foreskin is attached to the head to prevent stuff like poo from touching the head. So if it doesn't slide back, they'll rip it from the head (if it's attached) or make a cut across if it's tight.
