It takes 5-6 hours for me to ejaculate and if i dont then i have massive cramps and my testicles swell and cause extreme discomfort and pain

I'm guessing that it doesn't take you nearly that long to climax when you are masturbating. If that is accurate, then have intercourse until your partner is satisfied, and then finish yourself with your hand...or any other way that happens more easily for you.
Or maybe more foreplay on you or between the two of you prior to intercourse would be helpful.
Most ejaculatory problems (premature ejaculation being much more common) are psychological rather than medical. But I see no problem or shame in doing what you have to do to resolve the problem.
