Circumcision is done mostly for religious or cultural reasons. Many also claim to do it for hygienic or health reasons, but circumcision is very rarely medically necessary. Circumcision only offers true benefit to the very few who develop problems involving their foreskins that can't otherwise be cured.

In the vast majority of cases, there are no negative effects of not getting circumcised. This is largely dependent on two things: the maintanence of good genital hygiene (link 1) and safe sex practices. Good genital hygiene can reduce/eliminate most problems associated with the foreskin. Most studies find no significant difference between circumcised and uncircumcised men with respects to getting infections/STDs. Good hygiene synergistically further reduces risks for both groups (link 2).

There are some positive effects of not getting circumcised. The foreskin contains many nerve endings that may contribute to sexual sensitivity (link 3). Furthermore, several studies indicate that the foreskin may play a positive role in the sexual satisfaction/pleasure of not only the man, but the woman as well (links 4, 5, 6).

So as long as there's nothing medically wrong with a person's foreskin, he maintains good genital hygiene, and he practices safe sex, there are no real negative effects of not getting circumcised. In fact, there may be a few positive effects.
