Boys are born intact with a foreskin.
Circumcision rates vary by regions. The USA is the only advanced/industrialized nation that still does it a lot (the Japanese and Europeans don't do it), but our rates are going down, too. The West Coast has rates as low as 14%, while the Mid West has much higher rates. Generally rates have gone down over the years, though. And if Jewish or Muslim, yes, they're generally circumcised.
