Middle ear cavity is the area just behind the ear drum. it is part of the ear between the visible, external part and the inner ear ( part responsible for hearing, having numerous nerve connections) . It is normally filled with air and connected to nose through a tube, nose keeps this space filled with air.
Mastoid is the bone adjoined to it, and it is also made up of nuemrous air filled cavities.

during inflammation/ infection, like cold, etc, the air in this cavities becomes less ( as nose itself is blocked and unable to send air ), and the inner skin lining the cavity becomes swollen.
repeated infections in the mastoid cavity will make it opaque and devoid of air. On x ray often then these cavities look white instead of looking black.
mastioid cavities in different people vary in the amount of air it contains, some people it is normally opaque.
