My wife died of liver failure and she was not in any relative pain.
Early on in the disease their is abdominal pain and ascites (swelling of the abdomen) can cause breathing problems. But for her the only real pain was when her kidneys shut down and she had sever muscle cramps.
Mostly, patients in liver failure will sleep a lot then fall into a coma and pass away.

at the point of complete failure the patetint ill be sedated anyway, but i knew a guy who was dyeing of liver failure and he took straight gin and and ran it to his iv and his liver was shot, but he found it easier to take his own life this way as he was an alcoholic. and this was in a hospital. he had refused to be sedated,and went out drunk... a very sad story with a wife and 2 young kids

It can cause acute abdominal pain.

I work with a lot of patients in liver failure. It is not a pleasant thing to experience. It can be very painful as the toxins build up in your system. They often turn a bright yellow, and sometimes even green color! Granted my patients are in the ICU because they are so sick, but I would never want to experience liver failure!!
