The Black Death

(from mideival times, its a good one)

The Black plague comes to mind.

bubonic plague (aka black plague / black death). so famous there was a nursery rhyme about it (ring around the rosey, pocket full of posey, ...)

1918 flu epidemic

Perhaps the most famous plague epidemic in history is the Black Plague of fourteenth century Europe. However, plagues have struck repeatedly throughout history and continue to do so. In many ways much more devastating than the Black Plague was the influenza outbreak of 1918.

mad cow disease or the beobonic plague(a.k.a. the black death)

this is what you want

The polio epidemic in the 40s was really bad, and some of the survivors are having relapses.

Might tell people to get their kids immunized while you're at it.

how about typhoid fever or the black plague?

it seems that everyone thinks the bubonic plague. it's a good one, since there is so much to it, such as religious groups that formed around it, how hospitals managed, the decrease in population (naturally), how it began, and how some were saved (if you let the pus drain out, sometimes you would live; on man hid his family away and they survived).

it's a good one.
