I don't think that kind of information is in a good heath record.
For my program, I had to go to the doctor with a school-supplied form. It was a general check up with the usual question. Blood pressure check, urine analysis, hemoglobin and sugars check, breathing test, eyes, hearing...
Most likely, the school wants to make sure you're not going to have a stroke, heart attack, don't have any physical problems that would hinder you...
Why don't you just ask your school. I think they're supposed to tell you if you ask them. Just ask them, "what's in this good health record, anyway?"
