You can't compare them like that. Treatment for Diabetes goes in steps unless your pancreas does not function at all which makes you insulin dependent. Usually they try diet & exercise first, then oral meds, then insulin, in that order.

Just by following a simple diet plan with daily exercise, you might just be able to get off all medication completely. I did.

Some type 2 diabetics are so b/c they do not produce enough insulin. In these cases, oral hypoglycemics may not be as responsive as in patients who are insulin resistant and are producing enough insulin but not getting their blood sugar lowered enough b/c their bodies are resistant to the insulin. Oral hygpos work well in these patients to lower the blood sugar slightly. With some who do not receive enough insulin b/c they do not get enough to start. In these cases insulin works much better. However, for all type 2 diabetics who do not work on exercise and diet enough, oral hypos may not be enough and when you cannot control your blood sugar, you need more help. The longer your blood sugar is out of control, the higher the risk for problems. Be sure your dental cleanings are up to date b/c oral problems can make a big impact on blood sugar control.

Both are good, it depends how and for what type of DM type 1 or 2 they are used and is there any other problem such as active infection or not at the same time which is making the DM difficult to be controlled

Type 2 diabetes can result from a lack of insulin production, insulin resistance at the cellular level, or a combination of both.

Some oral hypoglycaemics increase insulin production, others increase sensitivity of the cell wall so that the insulin can be utilized.

If the diabetes progresses so that the oral drugs do not give control, then insulin may be the next step.

Also some type 2 diabetics require insulin at times of stress such as postoperatively or if they have a severe infections.
